Central Auckland TENS Hire

I agree to hire the TENS machine on the above date and return it on as soon as possible after the birth of my child (up to 14 days post birth).

The TENs machine comprises:

• 1x TENS machine
• 1x operation manual
• 1x set of lead wires and electrodes
• 1x set of self-adhering electrodes (yours to keep and dispose of)
• 1x “A woman’s guide to drug free pain relief” booklet.

Terms and Conditions:
1. I agree to use this machine in consultation with my Lead Maternity Carer.
2. The TENS machine should only be used in accordance with the instructions provided.
3. The success of a TENS machine in labour is not guaranteed by Parents Centre Aotearoa.
4. The TENS machine must not be used by anyone fitted with a cardiac pacemaker, who has a heart disease or suffers from epilepsy. If you have any concerns whatsoever consult your medical advisor prior to use.
5. The TENS machine cannot be used in water. If you wish to to enter a bath, birthing pool or shower, the TENS pads should be removed.
6. The TENS machine must not be used until the pregnancy has reached full term (37 weeks onward)
7. I agree that if the machine is returned in an unclean state I will forfeit my $60 bond.
8. I agree that if the machine is not returned for any reason, or returned broken or with pieces missing I will pay the replacement costs for the items in addition to forfeiting my $60 bond. (Tens Machine total replacement, $195, Operation manual, $5)
9. Non-use of the hired TENS machine is entirely at your discretion and no refund will apply
10. Payment constitutes hireage even in the event of non-collection of the TENS machine
11. You will need to provide your own batteries.
